Understanding SAD: More Than Just the Winter Blues

Seasonal Affective Disorder is a type of depression that follows a seasonal pattern, typically emerging in the autumn and winter months and resolving in the spring. It’s far more significant than simply disliking cold weather. People with SAD experience genuine depressive episodes that can drain their energy and make them feel moody and lethargic.

Common symptoms of SAD include:

  • Persistent low mood, sadness or hopelessness
  • Loss of pleasure or interest in everyday activities
  • Irritability and feelings of despair
  • Lethargy and a lack of energy during the daytime
  • Sleeping excessively (hypersomnia) and finding it hard to get up
  • Craving carbohydrates and gaining weight
  • Difficulty concentrating

The primary driver of SAD is the reduced exposure to sunlight during winter. This disrupts our internal body clock, which regulates sleep, mood and hormones. Harvard Health explains that a lack of light throws off our circadian rhythm, causing the brain to produce too much of the sleep hormone melatonin and too little of the “feel-good” chemical serotonin, leaving us feeling low and drained. This disruption can affect up to 5% of the population, with symptoms lasting roughly 5 months a year.

How Does Light Therapy Work? A Biological Approach

Conventional light therapy has been a first-line treatment for SAD for decades. Standard treatment usually involves sitting in front of a bright white light box (10,000 lux) for 30 minutes each morning. The NHS notes that this type of treatment can help regulate the production of melatonin and serotonin.

Key mechanisms of light therapy on the brain:

  • Regulating the Circadian Rhythm: Light is the primary signal for our internal clock. Morning light helps shift our sleep-wake cycle back to a normal phase, combatting the winter delay.
  • Suppressing Melatonin: Light signals the brain to stop producing melatonin, the hormone that makes us sleepy, helping us to feel more alert during the day.
  • Boosting Serotonin: Exposure to light can increase serotonin production in the brain, helping elevate mood and restore a sense of well-being.

A significant network meta-analysis published in 2024 in the Journal of Affective Disorders analysed 21 clinical trials involving 1,037 participants. It concluded that bright light therapy is a “promising” and “statistically significant” treatment for alleviating the symptoms of SAD. 

The evidence supporting light therapy is so robust that it is often compared to the effectiveness of antidepressant medication or talking therapies.

From Bright White to Red & Infrared: A New Contender

While white light boxes are the standard, recent years have seen a surge of interest in red light therapy (often referred to as photobiomodulation). Unlike the bright, glaring light of a SAD lamp, red light therapy delivers specific wavelengths (typically in the 630–850 nanometre range) that are gentle on the eyes and designed to interact deeply with our cells.

How red light therapy differs:

  • Glare-Free: The light isn't designed to wake you up through your eyes; it works on the tissues.

But can this “cellular energy” affect our mood? Emerging research suggests yes. A recent review examining fourteen studies up to July 2025 concluded that Photobiomodulation (PBM) has “therapeutic potential” for mental health issues, including SAD, with reported improvements in brain activity and antidepressant effects.

What the Research Says: Infrared's Surprising Advantage

One of the most fascinating findings in this field challenges the assumption that you need visible white light to treat SAD. A groundbreaking study, Prophylactic treatment of seasonal affective disorder (SAD) by using light visors, investigated whether light had to be visible to be effective. The study involved 38 SAD patients who started treatment in early autumn to try to prevent winter depression.

The groups were exposed to:

  • Bright white light
  • Infrared light (which is invisible to the human eye and was measured at only 0.18 lux)
  • No light (control group)

The results were remarkable: Infrared light was just as effective as bright white light. Both were significantly more effective than doing nothing. The authors noted that the fact that invisible infrared light worked as well as bright light “questions the specific role of visible light in the treatment of SAD”.

This suggests that the therapeutic benefits of light may not simply be about visual stimulation or circadian signalling, but also about the direct biological effect of infrared wavelengths on the body’s cells.

Exploring the Potential: Red Light and Mood Regulation

While the above study used infrared, what about the red wavelengths often found in home-use panels? Though more research is needed specifically for SAD, several trials on general depression and cellular health are promising.

  • Daily Sessions Improve Mood: A 2019 clinical trial found that daily 20-minute near-infrared light sessions directed to the forehead significantly improved mood scores among participants with mild to moderate depression. Using a similar transcranial approach, a 2017 pilot study reported improved mood and cognitive performance in healthy adults after just two weeks.
  • Rapid Response: A 2012 study published in the Journal of Nervous and Mental Disease looked at rapid effects. Patients with SAD received one hour of bright light and one hour of placebo red light. Even after a single session, there was a “significant but modest improvement” in depression scores with the active light—demonstrating that light can work very quickly to lift mood.
  • Cellular Energy and Brain Function: Researchers suggest that light therapy may influence levels of Brain-Derived Neurotrophic Factor (BDNF), a protein crucial for neuron health and neuroplasticity. By stimulating mitochondria, red light may help improve energy availability for brain cells, counteracting the chronic low energy that characterises depressive states.

A 2025 systematic review and network meta-analysis comparing white, blue, green, and red light noted that, based on the current literature, red light may show a strong placebo effect. The review indicated that white light appears most effective, followed by green and blue.

However, this does not negate the potential value of red light, especially given its unique mechanism of acting through the body rather than the eyes, and the strong findings from the infrared visor study suggesting that invisible light wavelengths are powerful agents against SAD.
